You need to leave your name, date of birth, name of the medication and the use of it. The GP will authorize your request (if possible), after which you pick up your medication at your pharmacy.

You can also call the practice to order your prescription. Or you can put your medicine box in the mailbox at the door of our practice.

Are you registered at Apotheek Westwijk, Apotheek Asserring, Apotheek Keizer Karel or Apotheek Middenwaard? During our holidays you can go directly to your Pharmacy to order your repeat prescription. So you don’t have tot call our replacer.
